6000 Heisley Road, 2nd Floor. 440-357-6677 Conveniently located inside the Mentor Heisley Racquet & Fitness Club, our Mentor office is the ideal location for physical therapy. In addition to 2200 sq. ft. of our own office space, AGM has full access to the club's free weight and strength machines, cardiovascular equipment, indoor track, and sport courts which can be used to evaluate and treat nearly any sports injury. AGM also offers a convenient aquatic therapy schedule, with access to the heated pool for injuries that require an aquatic environment.

6550 North Ridge Road, Suite 202. 440-417-0033. Our office in Madison is located on the second floor of the Huntington Bank building on U.S. Route 20. Approximately 1500 sq. ft. of exercise space which includes the ability to treat all injury types using combinations of weights, machines, resistance bands, as well as modalities such as traction, ultrasound, and electrical stimulation.
