Over the past two years, AGM has been providing home physical therapy for a wide variety of injuries and illnesses. AGM functions as a subcontractor to several area home care agencies throughout Lake and Ashtabula Counties. After AGM receives an order to perform a home physical therapy evaluation, a plan of care is established in which specific treatment is carried out to get the patient back to improved health. The vast majority of the follow up care is provided by the same licensed physical therapy assistant, which leads to great continuity of care and improved outcomes.
